If you'd like to get started, we just need the following information:

Email * Required

Phone / Alt Phone
Fax #

We also need confirmation on which control panel you want installed, and which OS, and if you have any special partitioning requirements.

Our default setup is CentOS5 32bit with the following partitions:
/boot 100M
/var 45GB
/usr 25GB
/home *remaining disk*
/ 2GB
/tmp 2GB
swap 2GB
